We are looking for someone who has a commitment to a strength based, person led approach model of care. As well as strong communication, administration, and computer skills, combined with the ability to be self-directed and work under limited supervision. This is an exciting opportunity to join our United Disability Care team based in either Robina, Qld or Tweeds Heads office, NSW.

The Support Coordinator will be responsible for connecting participants with formal and informal services and supports in their community in alignment with their goals and NDIS funding. The role will also require the Support Coordinator to assist participants in understanding and reviewing their NDIS Plans.

**WHAT YOU WILL BRING WITH YOU**
- Experience within the Disability and/or Community Services sector
- Working knowledge of NDIS / NDIA
- Current National Police Check
- Proof of Covid 19 vaccinations
- NDIS Worker Orientation Certificate (preferred)
- Current Drivers licence
- Demonstrated problem solving skills
- Strong written and communication skills
- Able to build rapport with clients, family members and stakeholders
