Client Liaison Officer

**Your Responsibilities**
- Ensure the enquiry and experience for potential clients is seamless and be able to provide preliminary information regarding service, service agreement and service fees
- Liaise with and support clients and family members to promote confidence in our care and service offering
- Maintain community and network referral relationships to facilitate enquiries
- Manage waiting lists and relationships with potential clients whilst on the waiting list
- Ensure a smooth and effective hand-over to the clinical care team - Booking of RN to complete WHS checks and Client care plan
- Act as point of contact for clients and family members after service starts and making sure any changes on care needs is communicated across the clinical and rostering team
- Ensure compliance with legislative updates within the Aged Care and NDIS Sector
- Monitor client satisfaction and feedback and use this information to inform service improvements and innovations
- Develop and implement engagement strategies to build strong relationships with clients, stakeholders, and their families.
- Build and maintain positive relationships with key stakeholders, including government agencies, non-government organisations, and community groups.
- Promote awareness of Reliant services and programs through effective communication and engagement activities.
**To be successful in this position you must have**:
- Proven experience in a customer or information provision role
- Demonstrated accountability for the quality of one’s own work;
- Experience in negotiating and resolving conflict
- Demonstrated experience in engagement, community development or customer service role, preferably in the Disability and Aged Care industry or a related field.
- Strong communication, interpersonal, and relationship-building skills.
- Ability to work independently and as part of a team.
- Highly organised, with excellent time management and prioritisation skills
- Current driver's license and access to a reliable vehicle.
- Ideally with a clinical understanding, experience, or basic medical knowledge
