**Position Title**: Service Delivery Coordinator
**Division**: Supported Living and Daily Activities
**Reporting to**: Senior Manager
**Delegation Level**: Non award
**Key Objective of the Position**
- To be responsible for all operational functions of the program including but not limited to:
- Person Centered support to clients and, where appropriate, their family members.
- Assess client needs and goals - support with the activities of daily living, accessing the community and actively working towards client goals.
- Responding to and resolving client enquiries/concerns and advocate for clients as needed.
- Development and implementation of staff work rosters and shift duties.
- Be flexible to modify own work hours to suit vacancies on participants rosters as needed.
- Ensure client appointments are met and followed up on/implemented by appropriate persons.
- Maintain accurate and timely case notes, records, and reports, & promptly follow up as necessary on identified needs.
- Provide Practice leadership to staff including:
- Conducting team meetings.
- One to one supervision and supporting with disciplinary meetings if required.
- Engage actively in recruitment, training, and appraisals.
- To work with the relevant Managers to develop and implement systems, policies and procedures that comply with legal and funding requirements, manage risk and continuously improve service and to ensure consistency in support and processes across all service areas.

- Collaborate with other professionals and team members such as Support Coordinators, Therapists, and the NDIA, as necessary.
- Listen effectively, convey and receive ideas, information, and direction - ensure communication (both written and oral) is clear and concise.
- Seek to clarify and confirm the accuracy of their understanding of unfamiliar or vague terms and instructions.

- Prepare sound proposal and seeks approval for any purchases outside of budget allocations.

- Make sound decisions regarding immediate service delivery within service specifications, relevant policies, and guidelines.
- Planning and organisation - make effective use of time and facilities to meet required deadlines.
- Maintain a clear grasp of daily tasks and prioritise duties/responsibilities in a manner consistent with service and organisational objectives. Delegate and follow up as required.
- Demonstrated experience working with people with a disability, including supporting people in a residential setting:
- understanding of 'customer centered approach'.
- working with behaviours and understanding PBSP's.
- manual handling - transfers and mobility supports.
- medication administration and support with care plans.
- planning activities, programs of support, working towards NDIS goals.
- In-depth knowledge of the Disability Sector including NDIS funding, SIL quotes, and the NDIS Quality and Safeguarding Framework.
- Proven ability to successfully lead and support a team of staff working in residential settings

**Secondary requirements for the role**
- Current drivers licence and reliable vehicle.
- Right to work in Australia.

- Computer literacy - Microsoft suite.
- Extensive experience in the industry or tertiary qualifications to suit this role.
**Job Types**: Full-time, Casual
Pay: $90,000.00 - $100,000.00 per year
