**Primary Purpose of the Position**

The Support Coordinator is to provide individualised support to participants (who have a disability and/or mental illness) of the National Disability Insurance Scheme (NDIS). The Support Coordinator will work with the participant to implement the supports within the persons plan for the purpose of building capacity, managing the impact of their disability, and improving their health, well-being and participation within the community.

**Position Overview**

As a Support Coordinator you will provide varying levels of coordination and transition supports for children, young people, and adults. Considering an individual’s circumstance and level of complexity, the role actively supports people to:

- research, coordinate and manage a range of supports to suit individual needs across multiple providers
- connect with their communities and encourages participation in both mainstream and community options
- coordinate and manage their plan to enable greater independence across all domains of an individual’s life

As a Support Coordinator your primary focus is to enable an individual:

- to build their capacity (and that of their networks) with the aim of independently managing their issues and supports whilst meeting their goals and exercising choice in obtaining supports that assist them in pursuing their own lifestyles.

**Key Roles and Responsibilities**
- Manage your own time, set priorities, plan, and organise your own quality work to achieve specific objectives
- High level knowledge and skills in coordination of supports (case management) to assist people with a disability in their community or in their home
- Excellent Understanding of the Disability Act 2006 and understanding of the policies and procedures generally associated with the provision of Disability Support Services
- Identify potential crisis situations and proactively manage and coordinate with mínimal support from manager
- Work within the funding levels associated with each participant (continually track and monitor billable hours)
- Takes responsibility for own work outcomes and assists others to understand role and responsibilities
- Develop and record progress of goals for participants that are specific, measurable, achievable, relevant and time limited (SMART)
- Continually gather and document relevant information about participant s from other services for the development of evidence-based reports and updates

**Collaboration and Community Connections**
- Liaise and work collaboratively with key stakeholders to establish a mutual goal of coordinating service delivery and developing effective linkages relevant to the achievement of the individuals NDIS goals
- Ability to effectively manage complex multi
- disciplinary teams and services in the coordination of a participant’s plan
- Coordinate external meetings with participants, significant others and providers and follow up any outstanding actions in a timely manner
- Develop and establish effective professional working relationships with funders and community organisations (mainstream and disability) that enable the delivery of participant outcomes
- Contribute knowledge in establishing procedures in the appropriate work-related field.

**Capacity Building**
- Empower participants to exercise choice and control to access a range of formal and informal mainstream and community-based support, by identifying opportunities for innovative service delivery
- Continually seek opportunities to build the capacity of participants and their families enabling greater levels of independence
- Develop and strengthen an individual’s formal and informal supports
- Support participants to engage with community partners, stakeholders to achieve a desired outcome
- Actively participate in the development, implementation, monitoring, regular review and reporting of participants programs with key stakeholders

**Selection Criteria / Key Competencies**
- High knowledge and understanding of the National Disability Insurance Scheme (NDIS)
- Awareness of disability sector and people with disabilities.
- Experience in providing services to participants as a Support Coordinator
- Experience in the assessment and monitoring processes for NDIS participants including other mainstream services
- Completed risk assessments in the workplace safety context or willingness to learn
- High level of verbal and written communication skills including skills for Microsoft Suite
- Commitment of working with participants within a strength based, person-centred framework
- Leadership skills and proven relationship management skills with internal and external stakeholders
- Ability to work effectively, efficiently, and accurately and prioritise workloads
- Problem solving skills and the ability to leverage networks
- Current Valid Driver’s Licence.
- Comprehensively Insured Vehicle.
- All applicants will need to have a valid NDIS Workers Screening Check.
