What We're Looking For:
Support Coordinator's ready to take on a full caseload.
Strong knowledge of the NDIS framework.
Excellent communication and organisational skills.
Ability to work independently while contributing to a team.
Relevant qualifications (Social Work, Psychology, Disability, Community Services, etc.) preferred.
About the Role:
As an
NDIS Support Coordinator
, you will play a crucial role in helping participants navigate their plans, connect with services, and achieve their goals.
Key Responsibilities:
Support participants
in understanding and implementing their NDIS plans.
Connect them
with the right services, health professionals, and community resources.
Monitor progress
, ensuring supports align with participant goals.
Maintain compliance
with NDIS guidelines while focusing on quality outcomes.
Work collaboratively
with a multidisciplinary team to deliver holistic, person-centred support.

Location:
You will be required to service community members requiring your support in the
Western / South Western Sydney
area, and be available to provide face-to-face support as required and when necessary.
