**About the Role**:
**Worthy Ability** is seeking a compassionate and dedicated **Key Worker** to join our team. As a Key Worker, you will play a crucial role in supporting individuals with disabilities, helping them achieve their goals, and enhance their quality of life through person-centered care. This role offers the opportunity to make a significant impact on the lives of individuals and their families.
In this role, you will work closely with clients, their families, and a multidisciplinary team to ensure high-quality support services are delivered in alignment with the **NDIS** framework.
**Key Responsibilities**:
- **Client Support & Advocacy**: Serve as the primary point of contact for clients, providing guidance and support in all aspects of their care and advocating for their needs.
- **Care Coordination**: Develop and implement tailored care plans that align with each client’s goals and NDIS funding requirements.
- **Family & Stakeholder Engagement**: Work closely with families, caregivers, and other professionals to ensure consistent, holistic support for clients.
- **Progress Monitoring & Reporting**: Track client progress, make adjustments to care plans as necessary, and ensure accurate documentation in line with NDIS standards.
- **Training & Support**: Provide training and support to families, carers, and team members in implementing behavior support strategies and other necessary interventions.
**Essential Skills & Qualifications**:
- **Relevant Qualifications**: A degree in **Social Work**, **Psychology**, **Occupational Therapy**, or a related field.
- **Experience**: Experience working with individuals with disabilities, ideally within the NDIS framework.
- **Communication Skills**: Strong interpersonal skills and the ability to engage effectively with clients, families, and team members.
- **Knowledge of NDIS**: Familiarity with NDIS policies, procedures, and individualized care approaches.
- **Problem-Solving Abilities**: Capacity to develop innovative solutions to support individuals in achieving their goals.
- **Team Player**: Collaborative attitude and ability to work in a multidisciplinary team.
