**How you’ll play your part to support individuals maintain choice and control in their homes**

You will be required to provide in-home and community support to people with mental health issues, disabilities, elderly individuals and those requiring community care. This may be through personal care, domestic assistance, respite, community access, meal preparation etc. Alongside this, having the responsibility for monitoring a client’s progress, reporting any concerns or changes and documenting accurate and relevant information in client files.

Your morning might start with an elderly client needing assistance with daily tasks, such as having a shower and preparing breakfast. You may continue to your next client who seeks to move towards greater independence and develop new skills through social support. Your day could finish with a client who is unable to drive, and so you take them grocery shopping and to any appointments.

**A bit about you**
- Are proactive with strong interpersonal skills for establishing relationships and dealing with client needs on a daily basis
- Hold a qualification in Individual Support, Aged Care, Disability or Community Care
- Must have access to a reliable car and valid Licence
- Have a current (or willing to apply) Police Check, Working With Children Check and NDIS Worker Check

Ability to commute/relocate:

- Parramatta, NSW: Reliably commute or planning to relocate before starting work (preferred)

**Experience**:

- support worker: 1 year (preferred)

Licence/Certification:

- First Aid Certification (preferred)
- Working with Children Check (preferred)
- National Police Certificate (preferred)
- Driver Licence (preferred)
- qualification in disability (preferred)

Work Authorisation:

- Australia (required)

Shift availability:

- Day Shift (preferred)
- Night Shift (preferred)
- Overnight Shift (preferred)

Work Location: On the road
