Are you passionate about making a real difference in the lives of older people? We’re looking for a dedicated Grade 2 Allied Health Assistant to join our dynamic Community Based Transition Care Program team and support the delivery of high-quality, evidence-based, and patient-centred care.

Working under the supervision of our Physiotherapists and Occupational Therapists, you will play a key role in assisting with therapy programs, supporting patients and their families, and contributing to positive health outcomes.

**Key Responsibilities**
- Assist in the implementation of treatment plans developed by Allied Health professionals.
- Provide support to patients, families, and carers in a compassionate and respectful manner.
- Use competent clinical support skills in accordance with delegated tasks and clinical guidelines.
- Communicate effectively with the multidisciplinary team to ensure coordinated care.
- Contribute actively to the goals of the Allied Health service and broader Community Based Transition Care Program team.

- Qualified Allied Health Assistant
- Experience in hospital or community-based healthcare,
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary team.
