We are seeking a compassionate and experienced Disabilities Services Officer to join our dynamic team in Phillip, ACT.
Position Overview:
As a Disabilities Services Officer at Greenleaf Care Group, you will be responsible for evaluating client needs and creating customized support programs. You will collaborate closely with clients, their families, and other stakeholders to ensure comprehensive and holistic support services are delivered.
Key Responsibilities:
- Determine client needs through interviews and assessments, and create personalized educational, training, and support initiatives.
- Track and report client progress, making necessary adjustments to support plans as needed.
- Connect clients with external agencies for supplementary support and resources when appropriate.
- Evaluate community requirements and resources in areas such as health, welfare, housing, employment, and training, and engage with relevant stakeholders to raise awareness of community resources and services.
- Offer family support and care for children and disabled individuals in adult service units, group housing, and government institutions.
- Oversee offenders on probation and parole, and provide guidance to young people facing social, emotional, and financial challenges.
- Develop funding and resource proposals and produce reports for government bodies and other organizations.
Essential Requirements:
- A minimum of 2 years of experience working in a similar role within the disability services sector.
- A relevant qualification in disability, community services, or a related field.
- A current First Aid Certificate and CPR certification.
- A valid Australian driver's license and reliable vehicle.
- A current Working with Vulnerable People registration, or the ability to obtain one.
- Excellent interpersonal and communication skills, with the ability to establish rapport with clients, their families, and colleagues.
- A strong commitment to providing person-centered care and promoting the rights and dignity of people living with disabilities.
Salary Range: $62,000 - $67,000 plus superannuation per annum
