We are currently seeking a Support Coordinator to join our team. This is an exciting opportunity for someone who is passionate about making a difference in people's lives and has a strong commitment to providing high-quality support services.

**Responsibilities**:

- Develop and manage individual support plans for clients
- Build strong relationships with clients, families, and other stakeholders
- Coordinate and allocate support services, ensuring that services are delivered in a timely and efficient manner
- Ensure that support services are meeting the needs of clients and are of high quality
- Monitor and evaluate client progress, making changes to support plans as required
- Manage support workers and provide supervision and guidance to ensure their professional development
- Respond to client concerns and complaints in a timely and professional manner
- Participate in team meetings and contribute to the development of support services

**Requirements**:

- A relevant qualification in a relevant discipline, such as community services, disability services, nursing or social work
- Experience in a similar role
- Knowledge of the NDIS and experience working within the disability sector
- Excellent interpersonal, communication and organizational skills
- Ability to work independently and as part of a team
- Strong problem-solving and decision-making skills
- Ability to manage competing priorities and meet deadlines
- A current Driver's License and willingness to travel
