We support people with physical, intellectual and multiple disabilities and developmental delays to achieve their goals Scope is a registered provider under the NDIS and delivers a range of services including supported independent living, short term accommodation (respite), lifestyle options and therapy across 100 locations in Victoria.

**About The Role**

Scope has an exciting and unique opportunity for a Support Coordinator to work within our NSW Support Coordination team.
- Establish Support Coordination in your regions
- Provide support co-ordination for NDIS participants who have complex and multiple disabilities
- Facilitate implementation of all supports identified in a participant’s plan
- Strengthen participants ability to navigate the NDIS and co-ordinate their supports
- Empower participants to access and co-ordinate supports to enable each person to participate more fully in their community of choice
- Develop strong local networks with service providers
- Assist participants to identify the range of suitable supports and services in order to meet their individual goals identified in their NDIS plan
- Work with the participant’s networks to ensure that they are effectively linked into funded supports and services
- Work with the participant to link them into appropriate mainstream services such as housing, employment, education and health
- Work with the participant to develop their skills and resilience

**About You**

We are looking for someone who has the following skills set:

- Tertiary qualified, or equivalent in a discipline related to disability supports or 2 years experience working within the disability sector
- Experience supporting adults and children with complex disabilities utilising a range of person centred service models
- Experience in support planning and / or coordination
- An understanding of the NDIS and individualised funding
- Ability to work with mínimal supervision
- A high level of organisational and time management skills e.g. data collection, reporting
- Ability to work with a variety of stakeholders
- Excellent written and verbal communication skills, including report writing
