**Correlate Connections** is an NDIS registered service provider in Campbelltown offering services such as Occupational Therapy, Speech Therapy, Psychology and Support Coordination.

**About the role**: We are currently looking for a **Full Time Support Coordinator** who will fit our organisational culture and wants to join our team.

We are looking for a passionate, engaging and tenacious person with a ‘can-do’ attitude.

You will be responsible for coordinating supports as articulated in the participant’s NDIS plan and will facilitate participant engagement with informal and mainstream services which will ensure the participant achieves their goals.

You will be assisting the participant to connect with NDIS supports and build capacity and resilience.

**In this role you will**:

- Assist National Disability Insurance Scheme (NDIS) participants in activating, interpreting, and implementing their NDIS Plans, with a strong focus on seeing participants move towards their NDIS goals.
- Work closely with NDIS participants and their families/guardians to connect with individual services and supports outlined in their NDIS Plan, ensuring they get the most out of their funding and achieve outcomes.
- Support NDIS participants to build capacity to coordinate their NDIS funded plans, navigate the system, negotiate for appropriate support and services, and connect with community support and mainstream services.
- Work with local community and mainstream supports to build relationships with key stakeholders.

**About You**:

- Excellent administration skills and the ability to interpret information guides, processes, and policy documents.
- Ability to work in a busy environment, work independently and as a part of a team.
- Skills in relationship building, conflict resolution, and negotiation.
- Innovation and creativity to work alongside people to achieve their NDIS plan goals.
- Previous experience as an NDIS Support Coordinator, Local Area Coordinator or similar.
- Have a strong understanding of complex family matters including child protection reporting requirements.

**Job Types**: Full-time, Subcontract

**Salary**: $65,000.00 - $80,000.00 per year
