**About the Role**

Rare Cancers Australia is embarking on an innovative initiative to establish a Telehealth Cancer Nursing and Navigation Program for individuals diagnosed with rare cancers. The Health Promotion Officer will lead this program's promotion and awareness efforts, ensuring its accessibility and impact on the target population. The ANCCP will provide remote access to specialised cancer nursing and navigation support for patients across Australia.

Reporting to the Head of Patient programs, your key responsibilities will include the following:

- Provide personalised support and guidance to individuals diagnosed with rare cancers, addressing their unique needs, concerns, and challenges.
- Assist patients in navigating the healthcare system, including coordinating appointments, accessing financial assistance programs, and understanding treatment options.
- Educate patients and their families about their cancer diagnosis, treatment options, and available support resources.
- Connect patients with relevant medical specialists, clinical trials, support groups, and community resources to enhance their care and well-being.
- Stay informed about advancements in rare cancer research, treatment modalities, and supportive care services to provide up-to-date information to patients.
- Offer compassionate listening and emotional support to patients and their families as they navigate the challenges of a rare cancer diagnosis.
- Provide referrals to counselling and coping strategies to help patients cope with the emotional impact of their diagnosis, treatment side effects, and uncertainties about the future.
- Collaborate with healthcare providers, multidisciplinary cancer care teams, and community organisations to coordinate comprehensive patient care plans.
- Enhance support services for rare cancer patients by building and maintaining relationships with key stakeholders, including medical professionals, researchers, patient advocacy groups, and government agencies.
- Maintain accurate and confidential records of patient interactions, including assessments, referrals, and outcomes, in accordance with privacy and confidentiality policies.
- Generate reports and data summaries to evaluate the effectiveness of patient support programs and inform strategic planning and decision-making.

This role is offered with a maximum term of 3 years aligned with government funding. However, opportunities for extension or permanency may arise during the contract term.

To be considered for this role, you must have relevant experience, skills, knowledge of the healthcare industry, and the required qualifications/certificates.
- Tertiary qualifications in public health, health promotion, communications, or a related field.
- Experience in health promotion, communications, or marketing, preferably in the healthcare or nonprofit sector.
- Strong understanding of health promotion principles, social determinants of health, and behaviour change theories.
- Excellent written and verbal communication skills, with the ability to craft compelling messages for diverse audiences.
