**We have an exciting Full-time Peer Community Support Worker opportunity available in Bowral & Greater Sydney.**

**The Housing and Accommodation Support Initiative (HASI)** is a state wide program that supports people with a severe mental illness to live and recover in the community in the way that they want to.

Services delivered under **HASI **encompass a wide range of activities that build independence in daily life. Supports can include recovery planning, support to live independently, assistance to manage a tenancy, assistance to manage health and emotional wellbeing, as well as support to get involved in your local community.

These positions involve shift work **Monday to Sunday rotating roster between 6am - 8pm **and you will need to be available to work flexible shifts that includes a combination of **mornings, evenings, and weekends.**

**Based in Bowral, and working across in Greater Sydney. You will report to the Team Leader in our Inclusion Team.**

**Key responsibilities will include**
- Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Collaboration with Community Mental health team around clinical goals set for customer
- Complete comprehensive risk management plans with customers
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required to achieve customer goals
- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date daily using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey

**You'll also need**
- Technical literacy with proven ability to use Microsoft Office and systems for reporting
- ** Minimum Cert IV in Mental Health Peer work** or Disability or equivalent industry experience
- Availability to work across a rotating roster
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
- Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us
