**About You**
- You are an Aboriginal and/or Torres Strait Islander person
- You have a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (overseas qualifications must supply the AASW Overseas Assessment number) and the maintenance of currency of this.
- Unencumbered driver's license and ability to travel across the District and to other external sites as required

**What you’ll be doing**

The position of the Aboriginal Social Worker exists within Nunyara Aboriginal Health Unit, as a part of Central Coast Local Health District.

The position is a clinical and community support role in the Aboriginal Health Team. This role is responsible for the delivery and monitoring of Social Work services to clients referred to the Aboriginal Chronic Care service. Referrals are from the acute and community sectors for clients with acute or chronic conditions, and their carers, requiring Social Work support, assessment and intervention.

The Aboriginal Social Worker will provide a professional effective social work service to patients and their family/carers to ensure the service is culturally safe. Primarily the position is to provide assessment and counselling to assist our Aboriginal and Torres Strait Islander community to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services.
- The role will work closely with the whole Aboriginal Health Unit and a range of other professions, support services in hospital and community to ensure the development and delivery of effective and pro-active social work services. The Aboriginal social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.
- Aboriginal Social Work services are provided in the community setting involving cultural integrity, during home visits, clinic and group interventions within the Aboriginal Chronic care team.
