**About You**
- You have a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number) with demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base and current drivers licence.
- You possess well-developed understanding of the psycho-social impact of acute and chronic health conditions and in the range of related social work interventions required in community social work clinical practice.
- You have demonstrated effective verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences and modalities (individual, family and groups), with a commitment to professional development, evidence-based practice and continuous improvement.

**Where you'll be working**

The position of Social Worker is part of a community based multidisciplinary allied health team. Community Allied Health provides a community service approach seeing clients and their families across the Central Coast. This position will work within a dedicated interdisciplinary team to deliver high quality care in a dynamic team environment.

Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.

This position is required to work collaboratively with other social workers within the team to ensure appropriate and timely service provision across all geographical sites and services. The clinical duties are performed across multiple sites within CCLHD. There may be a requirement to work weekend or public holiday shifts, if required.
