Join our dynamic team at Central Coast Local Health District (CCLHD) providing outpatient Renal Social Work services based primarily at Long Jetty Community Health Centre and be a crucial part of delivering optimal health outcomes and psychosocial care.

With a deep understanding of the psychosocial challenges that accompany chronic and acute illness, you'll provide comprehensive assessments, crisis management, and brief interventions tailored to meet the unique needs of each individual and their support network. Your ability to adapt communication styles, work collaboratively in multidisciplinary teams, and prioritize competing demands will ensure the delivery of high-quality care in a fast-paced environment.

This Renal Social Work position will enable you to further your current knowledge and gain a broad breadth of community based Social Work practice. From providing emotional support, working with families and carers to facilitating smooth transitions through the healthcare system, you'll play a pivotal role in ensuring patients and their families receive the support they need during difficult times.

Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The Social Worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and pro-active Social Work services. The Social Worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.

**About You**
- You will have a tertiary qualification in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to Inpatient Social Work.
- You have knowledge of providing social Work services in a health service setting with an interest in the area of renal and/or chronic illness.

**Where you'll be working**

The Social Worker position is part of a community based multidisciplinary allied health team located primarily at Long Jetty Community Health Centre and also from time to time at Lake Haven Community Centre.
