**About the organisation**

Home Caring St George has been supporting people in the community for many years. Home Caring St George is a leading Aged Care Service provider dedicated to providing seniors with high-quality in-home aged care services. Providing various home care services across the Sydney metropolitan areas, we believe in empowering our clients with equity and care by providing accessible and quality aged care services.

**Duties**:

- Conduct initial assessment and prepare necessary care plans for clients.
- Formally review client care plans on an annual basis to ensure compliance to standards of care for quality review.
- Monitor, evaluate and review care plans as required by client care needs.
- Liaise with and refer to allied health, dementia and continence services and other medical practitioners as required.
- Advise rostering team of client needs.
- Ensure all services are addressing client needs according to care plan.

**Requirements**:

- Relevant qualifications in Care with Community experience e.g. Diploma in Home and Community Services, Care management or Nursing.
- Demonstrated experience in a community/ nursing role.
- Coordinating assessments & care planning.
- Strong problem-solving and leadership skills.
- Experience in care management and the Aged Care Quality Standards
- High level of organisational skills and details orientated.
- Excellent verbal and written communication skills.
- Bilingual skills are essential with Vietnamese, Cantonese or Mandarin as a compulsory language.
- Valid Australian driver’s license with access to own vehicle.
- Competent computer skills in Microsoft Office software and ability to learn business systems quickly.
- Demonstrate familiarity with Home Care packages, having previous experience in a home care environment will be a bonus.

**On offer**:

- Salary of $70,000 to $75,000 per year, plus Super and Car Allowance (dependent on experience).
