**Job Description**:
We are seeking a passionate and dynamic Support Coordinator to join our team. This role is ideal for both experienced coordinators seeking to deepen their impact in the disability sector and for developing coordinators eager to grow their skills in a supportive environment. As a Support Coordinator, you will play a pivotal role in enhancing the lives of NDIS participants by designing, implementing, and monitoring tailored support plans.

Key Responsibilities:
Collaborate with NDIS participants to understand their goals, strengths, and challenges.

Develop, implement, and review individualised support plans in alignment with NDIS guidelines.

Coordinate a range of services and supports, including liaising with service providers and community resources.

Monitor the effectiveness of support strategies and make adjustments as needed.

Maintain accurate records and documentation in compliance with organisation and NDIS policies.

Provide guidance and support to participants and their families, empowering them to make informed choices.

Qualifications:
Relevant qualifications in Disability Services, Social Work, Psychology, or related fields.

Experience in a similar role is highly regarded, but not mandatory.

Strong understanding of the NDIS framework.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.

Current driver’s licence and willingness to travel as required.
