We are looking for an experienced Team Leader to support our Aged Care Support Facilitation team and clients. You will be responsible for leading our Support Facilitation team across three regions, as well as managing a small caseload of clients. You will play a significant role in the attraction and retention of new and existing clients and will provide a consistent entry point for new home care package enquiries and orienting the consumer through CatholicCare's services in a consistent approach. You will also provide quality leadership that will enhance staff skills and competencies through a strengths-case approach.

This is a permanent full time position, consisting of 38 hours per week and adopts a hybrid model of work.

The salary range for this position is from $89,000.90 to $97,233.37 per annum + Superannuation + Salary Packaging.

**Our requirements to provide care**
- Relevant tertiary qualifications inn Aged, Health or Community Services and/or Business management, Certificate IV in Leadership or working towards
- Minimum 3 years' experience in leading a team
- Significant experience within the Aged Care sector
- Has received three doses of an approved COVID-19 vaccine; or has been issued with a Medical Contraindication Certificate on the prescribed NSW Health form, exempting from vaccinations
- Sound working knowledge and experience in operationalising Aged Care Services within the community and a thorough understanding of the My Aged Care system
- Complex case management experience
