**A bit about the role**

The Health Outreach Coordinator is responsible for managing our street-based health outreach program operating Wednesday - Sunday nights in Melbourne’s CBD and our primary health service on Saturdays. As part of the role the coordinator will be participating in the outreach roster to provide on-the-job training, clinical leadership, and will coordinate day-to-day operations for our night health-led outreach service and at our daytime Saturday Clinic Primary Health Service.

The position will be critical to develop and provide a health-focused outreach response which engages service users to create linkages with our Living Room Primary Health Service and a range of external health, outreach and housing services. The Health Outreach Coordinator will lead the development of high-quality clinical care, service and referral partnerships, and professional development and clinical supervision.

The Health Outreach Service team utilises an assertive outreach model to assist adults with complex health needs who are primarily vulnerable, sleeping rough, living in insecure accommodation or accessing services in the City of Melbourne.

**Key Requirements**
- Demonstrated experience (2+ years) in a community health setting and demonstrated experience in delivering clinical interventions to disadvantaged populations.
- Recognised tertiary qualifications in relevant health (preferably nursing) or human services related disciplines.
- Experience in developing partnerships that deliver improved referral pathways and outcomes for clients.
- Demonstrated experience in leading a team of client focused, resilient and high performing staff.

You’re an engaging and inspiring leader in the health sector that is ready for their next challenge. You come with qualifications (in relevant nursing and/or health related disciplines) and experience building a dynamic team that works on the front line.

You have experience in developing partnerships that deliver improved referral pathways and outcomes for clients. Your passion for achieving health and wellbeing outcomes for people experiencing or at risk of homelessness is contagious. You understand the importance of balancing client outcomes, clinical governance and developing a strong team culture, all while keeping the multidisciplinary service humming along.

You are able to prioritise tasks, support your team and coordinate service delivery, all while balancing lots of plates in the air. You know the importance of ethical and emotionally intelligent leadership practices and responsive thinking to help us provide high impact support, without judgement

You can switch from supporting a service user to coaching a team member. You can flex your approach up and down. You know the importance of boundaries and you tailor your support based on the needs of the people you are working with. Changing lives is your passion, but you still embrace rolling up your sleeves and mucking in to get the job done.

**A bit about us**:
The Living Room is a primary health service, the team works with people aged 18+ to provide free healthcare and support to improve the physical, mental and social well-being of individuals who are, or at risk of experiencing homelessness.

The only service of its kind in Melbourne, focuses on the social determinants of health to look beyond disease or illness, to see a person's overall situation. Our multi-disciplinary team works with individuals to identify goals and focus on harm minimisation, self-determination, and empowerment. Our Health Outreach Service is an extension of the primary health service and aims to engage people sleeping rough and connect them with preventative and ongoing health care.

We are a non-judgemental service with a strong focus on respect, client centred care and harm prevention. We work with clients on a personalised, tailored basis to address highly complex issues faced by people experiencing multiple and complex needs.

Youth Projects is an independent, registered charity providing front line support to young people and individuals experiencing disadvantage, unemployment, homelessness, alcohol and other drug issues. We recognise the complex and varying needs of the people we work with and go above and beyond, 365 days a year, to provide pathways out of poverty.
