- Permanent full-time role
- Dual lifestyle and carer role in a family owned residential aged care faciltity

**Key Responsibilities**
- To facilitate a great lifestyle and improve and preserve the quality of life for our residents through safe and suitable group and individualised activities to residents.
- Maintain accurate documentation of activities including evaluation of individual program plans and individual resident plans
- To know your residents well enough to know their preferences, and be able to promptly recognize and act on changes in health conditions.
- This is a dual role and you will also be responsible for the daily care, safety and well-being of our residents.
- To support the Registered Nurses to deliver individualized care through following care planning and support planning.
- Maintain and facilitate clear and consistent communication with families, external service providers and other relevant stakeholders.
- To lead by example always by following policies, procedures and management work instructions and helping us to deliver on our mission.
- Abide by the Code of Conduct.
- Stay up to date with training.
- Any other duties as required.
- Positively contribute to our plan for continuous improvement, with your feedback we make things better

**Minimum Qualifications**
- Certificate IV in leisure and lifestyle.
- Certificate III in aged care or disability support is desirable.
- Current first aid and CPR certificate are desirable.

**Skills and Experience**
- At least 2 years experience in a Lifestyle Role and/or Personal Care Worker or Assistant in Nursing in residential aged care.
- You have worked with people with dementia.
- Reliable and committed to the Forest Lake Lodge family.
- Proficient written and verbal communication skills in English.
- An energetic and dynamic person with effective time management skills.
- You will be a quick learner and proficient with basic IT systems.
- Vietnamese language would also be desirable.
