An exciting opportunity currently exists for an enthusiastic and highly experienced **care manager** to join our Health Independence Program (HIP) Central Intake and Fast Track Care Management Team.

**Details of position/position requirements**:
**This is an exciting opportunity for experienced Allied Health Professionals or Division 1 Registered Nurses to work in an energetic and high performing multi-disciplinary team supporting clients to discharge safely and to prevent hospital presentation or admission.**

**You will have great communication skills and broad knowledge of the health system genrerally. The team is diverse creating lots of opportunity to expand your skills.**

As a fast track care manager, you will be responsible for assessing clients post discharge and organising support and follow up so they remain safely in the community.

In your role as intake clinician you will have responsibility for triaging referrals for most of the Austin HIP services. This is an interesting role and you will make sure clients get the right service at the right time.

The position is office based.

We have 2 permanent part time positions available - 3 days, and 4 days a week.
