An exciting opportunity currently exists for an enthusiastic and highly experienced **Fast Track Care Manager** to join our **Health Independence Program (HIP) Central Intake and Fast Track Care Management team.** **HIP comprises a number of programs and specialist service to support clients safely in the community.**

**As a care manager you will play a critical role in supporting hospital flow by working with vulnerable clients at risk of hospital admission or representation. Our clients and their families/carers have varied needs. The work is always interesting and you will be well supported by our energetic multidisciplinary team of health professionals both within the health service and in the community.**

**To be successful in this role you must have excellent communication skills, experience working in a multidisciplinary team, knowledge of community support options and the aged care system. We are looking for an enthusiastic and experienced care manager to join our team.**
