Under the service name "Intercept", we provide early intervention and prevention for young people and their families in the Moreton Bay region. Our teams cover family support, early intervention youth homelessness, mental health outreach support, disability services, support services for young people in case, alternative education and training and high school-based youth support.

**About the role**:
Based in Caboolture, the role of Youth Mentor, is to empower young people aged between 12 - 25 living with a disability to increase their social skills, community connections, life skills and independence by providing one-on-one mentoring and support. This is a Community access role.
**In addition you will**:
Provide mentoring support and serve as a positive role model supporting young people to engage with the community
Facilitate small groups to promote social connection & skill development
Organise activities that assist young people achieve their identified goals.
Provide flexible service provision that is client-centred using an outreach model
**About you**:
To be successful in this role you will hold a relevant tertiary qualification in a human services field and/or experience working in a similar role with vulnerable youth.
**In addition you will**:
Maintain a current working with children check (blue card) and NDIS Clearance.
A current Australian driver’s licence.
Have excellent written and verbal communication skills
Possess the ability to work autonomously.
