About the Role
The Youth & Family Practitioner is a key member of the Staying on Track program, which supports young people transitioning from youth detention back into community, as well as those at risk of re-entering the justice system. This role focuses on providing intensive, wrap-around support that empowers young people to make positive life choices, re-engage with education and employment, and strengthen their cultural identity and family connections.
As a Youth & Family Practitioner, you will work directly with young people, their families, and community networks to deliver case management, mentoring, advocacy, and practical assistance. You will facilitate access to education, training, employment, housing, health, and cultural supports while helping to develop individualised plans that respond to each young person's unique strengths and needs.
The role requires a balance of office-based work, outreach in homes and community settings, and collaboration with multidisciplinary teams, Elders, schools, and statutory agencies such as Youth Justice. Sensitivity to complex trauma, a strengths-based approach, and cultural competence are essential, as is the ability to build trust and maintain strong relationships with young people and their families.

About You
Essential
- Proven experience in providing outreach and in-reach casework, advocacy, and practical support to young people and families with complex needs.
- Strong knowledge of Aboriginal and Torres Strait Islander cultural frameworks, family systems, and practice approaches, with particular expertise in youth justice and trauma-informed contexts.
- Demonstrated ability to develop, implement, and review case plans or Service Response Plans in partnership with young people and their families.
- Highly developed interpersonal skills, including the ability to build rapport, communicate effectively, and practice with cultural safety, trauma awareness, and a strengths-based approach.
- Skilled in collaborating with statutory and non-statutory agencies, schools, RTOs, Elders, and community partners to deliver coordinated and culturally safe support.
- Strong organisational and administrative capability, including time management, accurate record-keeping, and compliance with reporting requirements (e.g., SRS system, case notes).
- Capacity to remain calm and exercise sound judgment in high-stress or crisis situations, ensuring appropriate responses to risk and escalation.
Desirable
- Lived experience within community, youth justice, or related fields.
- Experience delivering culturally grounded, family-inclusive practice models, such as Family-Led Decision Making.
- Facilitation skills, including the ability to lead group sessions or yarning circles and support community education or engagement activities.
- Practical knowledge and application of trauma-informed, strengths-based, and restorative practices.
