**This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks' notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.**

**About the Role**

This part-time role, based at the Gosford Health Services Building within Gosford Hospital, works Wednesday to Friday from 8:00am to 4:30pm and may involve travel across the district and to other locations as needed.
We are seeking a dedicated and compassionate professional to join the Central Coast Local Health District as the Intellectual Disability Health Service Coordinator & Disability Pathway Navigator. This unique role combines clinical leadership and system navigation to improve health outcomes and service access for people with intellectual disabilities and other disabilities across the local health district.
In this vital role, you will provide expert advice and support to healthcare staff to deliver person-centred, inclusive care to people with intellectual disabilities and their families or carers in a variety of settings, including hospitals, outpatient clinics, and community health. You will lead the development and implementation of best clinical practices and provide clinical leadership that models high standards of care tailored to this vulnerable population. Working collaboratively with the Hunter New England Specialised Intellectual Disability Health Team and key internal and external stakeholders, you will build capacity within local health services and support the delivery of multidisciplinary outreach services. These services will include comprehensive assessment, care planning for people with complex needs, and tailored education to equip mainstream clinicians with the skills and confidence to deliver appropriate care.
The role will also play a critical role in navigating the National Disability Insurance Scheme (NDIS) landscape within the health system. You will work closely with the directorate of Clinical Operations to develop systems, pathways, and training that assist staff in supporting people with disabilities, their families, and carers. Your role will include facilitating clinical and systemic escalations, identifying and addressing barriers to care, and promoting continuous improvement of NDIS-related processes and disability inclusion practices.

**About You**
- You hold a relevant tertiary qualification as a Registered Nurse or Allied Health Professional, with current AHPRA registration and recent experience in delivering or managing clinical services within healthcare settings, particularly supporting people with intellectual disabilities.
- You possess strong communication, negotiation, and facilitation skills, with the ability to build therapeutic relationships, deliver education, and collaborate effectively with consumers, disability service agencies, and multidisciplinary teams.
- You have a thorough understanding of disability-related legislation, policies, and public health practices, and experience leading change management and developing policies or pathways to improve patient-centred care.
- You demonstrate excellent analytical and problem-solving abilities, including data evaluation, report writing, and providing authoritative advice to senior management and stakeholders to support service improvements.
