AS A SUPPORT COORDINATOR YOU WILL
- Build strong relationships with clients, their families and service providers.
- Take a holistic approach to help clients address their barriers to achieve NDIS goals.
- Build a client’s capacity to understand their funded supports, utilise their NDIS plan and plan/track budget expenditure.
- Support a client to negotiate service agreements.
- Build a client’s capacity to understand the NDIS funded supports and mainstream providers available in their local area.
- Coach and support clients through scheduled and unscheduled plan review processes with NDIS.
- Complete ad-hoc tasks.

ABOUT YOU
- A minimum of 12 months’ experience working as a Support Coordinator, Case Manager, or relevant field
- A minimum of Diploma in Community Services or relevant experience.
- Valid drivers’ licence, First Aid Certificate and NDIS Worker Screening Check.
- Valid ABN and insurances.
- Experience handling complex situations and ability to resolve issues.
- Strong communication and interpersonal skills.
- Has attention to detail and is organised.
- Self-motivated and able to work autonomously.
- Friendly, approachable and able to work in a team environment.
- Most importantly passionate about helping individuals achieve their goals.
