**About the role**Managing the Chronic and Complex Care Service, including heart failure, endocrine/diabetes, pain service, co-ordinated care outpatients and COPD Services.Actively contribute to the overall strategy and leadership within the clinical program, providing sound financial, administrative, operational and human resource management for all clinical services within the portfolio.Direction, co-ordination and supervision of staff within the Chronic and Complex Care team to ensure safe and efficient clinical practice.Proven ability to coordinate multidisciplinary teams, ensuring high-quality, patient care across a range of complex conditions and health care settings.Strong critical thinking, decision-making, and problem-solving skills, with the capacity to manage and prioritise.

**About you**Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) or an Allied Health professional with relevant post graduate qualification in management and/or equivalent experience.Demonstrated high level leadership within the workplace; ability to plan, implement and evaluate local service delivery and to form productive working relationships.Highly developed communication, negotiation, facilitation and conflict resolution skills.Demonstrated experience managing operational, financial, human resource & workplace health and safety considerations.Understanding of the role and function of community health services, with ability to comprehend the navigation of a patient's journey across hospital, community and primary care services.
