**MISSION**

**ROLE AND RESPONSIBILITIES**
- Providing support in community-based settings, group homes, respite, participant’s homes
- Providing support based on a person-centered approach, supporting inclusion and participation
- Providing support to assist clients with daily life tasks as well as community participation
- Working effectively both in a team and with different client ratios such as 1:1, 1:3 etc
- Building and maintaining positive relationships with stakeholders
- Being available to work a variety of shift types, including early morning, afternoon, evening, weekends, sleepovers and active nights
- Creating and complying to an Individual Support Plan
- Complying with all recording requirements, including but not limited to: recording medication administration, completing incident reports, completing participant progress notes, updating participant support plans and accounting for cash expenditure
- Attending and participating in scheduled supervision meetings with Service Manager or Operations Manager
- Ensuring own hours worked are accurately recorded and reflect actual hours worked
- Taking reasonable care for your own health and safety and for that of others in the workplace by working in accordance with legislative requirements
- Adhering to all Sunny Pine Home Care policies and procedures and NDIS guidelines and regulations
- All other duties as directed by Management
- Work a range of different shifts including day, evening and overnight.
- Travel to assist participants with services in group homes, personal homes, facilities and more

To be eligible to accept this role you will have provided the following:
NDIS Workers Screening Check (includes National Police Check)
International Police Check (where applicable)
Evidence of current Working with Children’s check (where applicable)
Evidence of a current Driver’s License (International or Australian whichever is applicable)
Signed and witnessed Statutory Declaration Form
Evidence of relevant Qualifications
Evidence of your right to work in Australia (Visa, unless you are an Australian Citizen or Permanent
Resident)
Current First Aid Certificate - Level 2
NDIS Worker Orientation Module Certificate
A smart phone device

Pay: $72,000.00 - $90,000.00 per year
