**Support Coordinator - Empower Lives, Build Community**

**Location**: Head Office - Parramatta, NSW

**Job Type**: Part-time or Full - time

We’re seeking an experienced Support Coordinator to join our close-knit team in Parramatta. This role is more than coordination — it’s about walking alongside people with disability, amplifying their voice, and helping them navigate the NDIS with clarity, confidence, and care.

**What You’ll Be Doing**
- Build genuine, trusting relationships with participants, families, and support networks
- Help participants understand their NDIS plan, funding, and available supports
- Connect clients with appropriate services, allied health professionals, and community supports
- Support psychosocial recovery and goal achievement through tailored care planning
- Facilitate assessments to determine funding needs (e.g. complex home modifications)
- Coach and empower clients to build confidence and make the most of their plan
- Maintain regular communication and respond to feedback with a commitment to improvement
- Prepare and attend plan review meetings, including reports and goal setting
- Uphold confidentiality and adhere to all organisational policies and WHS standards

**What You’ll Bring**
- Minimum 1 year experience in a similar role with solid understanding of NDIS guidelines
- Certificate IV in Disability, Allied Health qualification, or relevant sector experience
- Excellent communication and interpersonal skills
- Ability to problem-solve and overcome complex challenges
- Current Australian driver’s licence and access to your own vehicle

Pay: $43.00 - $48.00 per hour
