**Qualifications & experience**
- Diploma or higher qualifications in disability support, community welfare, social work, psychology or counselling
- At least 3 years experience in disability support, welfare/social work, welfare program management or related field
- Work cooperatively with team members in a productive way and follow instructions given by Clinical Lead
- Provide thoughtful, honest feedback to team members and managers and manage conflict in a productive way
- Good understanding of disability support services providers in the local area and good understanding of issues around mental health
- Good understanding of the community sector and Not for Profit organisations
- Good understating of relevant privacy and confidentiality in relation to customers
- Good understanding of what is considered appropriate self-disclosure, empathetic and compassionate towards clients
- Be able to communicate concepts and ideas in a way that members can easily comprehend

**Tasks & responsibilities**
- Working with clients to achieve their goals in line with this NDIS plan and Providing ongoing support coordination services to clients in line with their NDIS plan
- Using a coaching approach to assist clients in capacity building and maintaining accurate and up to date case notes on clients progress
- Engaging with clients, their supports and third-party organisations in a professional manner
- Undertaking work in line with the organisations policies, procedures and values and complying with all relevant legislation and codes of conduct
- Maintaining an up to date knowledge of the NDIS systems and practices and clear professional boundaries with clients
- Maintaining an up to date knowledge of local services and organisations clients can utilise and engaging and coordinating service providers on behalf of clients and link clients to relevant providers
- Meeting with and encouraging clients to develop and refine their goals
- Engage clients informal supports (family, carers, etc)
- Working with customers to help build their own capacity and independence, providing crisis support to clients and assessing and managing risk in line with organisations policies and procedures
- Engaging with NDIA staff and LAC’s
