About the Role:

Due to the expansion of Zest Project Co, we are looking for an experienced Support Coordinator or Specialist Support Coordinator to join our growing team. The successful candidate will play a crucial role in facilitating and coordinating support services for NDIS participants, ensuring they receive the assistance they need to achieve their goals.

Key Responsibilities:

Support Coordination: Collaborate with NDIS participants and their support networks to identify goals and needs, develop and coordinate tailored support services.

Participant Engagement: Foster trust and empower participants by promoting active involvement in decision-making.

Resource Management: Organise appointments, coordinate support worker services, and manage resources to meet participant needs while adhering to budget plans.

Progress Tracking & Reporting: Maintain accurate records and provide regular progress updates to ensure participants are on track to meet their NDIS goals.

Communication & Collaboration: Maintain open communication with participants, support workers, and external stakeholders, ensuring smooth service coordination.

Quality Assurance: Monitor the quality of support services and ensure they meet NDIS guidelines.

Working Conditions:

Flexible work arrangements are available including Work from Home.

What you need:

  • Experience in support coordination, with strong knowledge of the NDIS framework.
  • NDIS Worker Screening Check
  • NDIS Worker Orientation Module – 'Quality, Safety and You'
  • Current driver's license and reliable transport
  • Excellent organisational and communication skills.
  • Ability to work both independently and collaboratively.
  • Problem-solving and decision-making abilities.
