**About the role**:
In this role, you will be responsible for coordinating our clients and their assessed needs and goal plans.

You will ensure that care needs are being met through high-quality service delivery.

Your tasks will include supporting clients in developing or reviewing their Client Goal/Care Plan, coordinating and monitoring the delivery of services, maintaining appropriate communication with carers and family members, and mentoring home care workers in the community.

**This is Full Time role**
