**Salem Home Care** has its administrative base in Toowoomba - this will be your home base while you work with the local community.

**About the role**:
The Home Care Coordinator is responsible for the coordination of our clients, their assessed needs and goal plans. You will ensure care needs are being met and through high quality service delivery. Your responsibilities and tasks may include, for example:

- Support clients to develop (and/or review) their Client Goal/Care Plan
- Coordinate and monitor the delivery of services outlined in the Client Goal Plan both by Lutheran Services staff and external service providers
- Monitor the client’s satisfaction with the services and adjust the service mix to ensure a high degree of satisfaction
- Maintain appropriate communication with carers, family members and advocates nominated by the client
- Supporting and mentoring home care workers in the community
