**About this opportunity**

A Care Manager is responsible for delivering case management that is personalised to our clients, enabling them to reach their goals with their best interests in mind.

This is a role where you are able to engage with your clients over the phone and in person to conduct care consultations, client onboarding, care plan reviews and coordinating with other office staff to ensure the best possible service is provided.

**Key Responsibilities**:

- Partnering with the client to deliver collaborative care, managing clients budget and ensuring that we deliver exceptional customer service and being proactive in managing clients needs
- Actively communicate with team members, clients, family and other stakeholders to provide appropriate care and share any relevant information including feedback about caregiver performance
- Managing ongoing relationships with clients and their families through face to face meetings to discuss and review care plans and goals
- Ensuring our risk and quality management systems are implemented and audit our processes to ensure our compliance is to standard.

**About you**:

- Previous and relevant experience working with ageing adults or aged care
- Home Care Package experience and understanding of legislation and regulations for home care services
- Clinical governance qualifications or experience are highly desired
- Highly efficient and organized with proven time management skills
- Able to work under pressure and multitask shifting priorities
- Strong written and verbal communication skills

**Australian work rights & Driver's Licence**
