We are an approved provider that specialise in self-management and partner with our customers to create a support plan to meet their needs and optimise their funds. Our approach keeps people in control of their package, funding, and support.

**Your Role**

Here at HomeMade, your role will be a little different to that of the traditional Case Manager within the Home Care Package (HCP) space. To reflect this, we also refer to these roles internally as our
**'Support Partner'**.

As the Team Lead, your role will report directly to the
**Clinical**
**Manager **and you will be responsible for leading the delivery of high quality support and experience to HomeMade customers by fostering good relationships with customers, and their support network while providing consultation, advice and case management support.

Additionally, you will guide and direct the NSW Support Partner Team towards achieving operational goals, efficiency targets and overall team performance.

Given HomeMade is a relatively young business offering something new in the HCP marketplace, you will have the ability to identify opportunities for service innovation and take a proactive approach to implementation whilst the business continues to grow rapidly - and
**we aren't kidding** when we say rapidly.

**_Some of your responsibilities will include and are not limited to:_**
- Supports and leads the Support Partner Team by providing relevant feedback, coaching and training to direct reports, and by facilitating the performance management process.
- Provide strategic, tactical and operational change management support to the Team.
- Motivate, track and drive the Team to meet business goals.
- Collaborate with the Clinical Team & the Clinical Manager to create goal orientated support plans for Customers, through appropriate use of cost containment strategies and budget management, ensuring government and best practice guidelines are being met.
- Uphold delivery of services in accordance with HomeMade policies and procedures, Aged Care Act and Aged Care Quality Standards.
- Deliver strategic initiatives that drive Customer and business value, and leverage data to provide insights into the performance of our frontline teams.
- Implement quality improvement and risk management measures into all aspects of service including policies and standard operating procedures.
- Support your team to recognise their customers’ existing capabilities that support their self-management journey and in turn build on these.

To be successful in this role, you will need to be someone who is passionate about self-management and the space we work in (Home and Community care) whilst wanting to make a difference to peoples’ lives. Having prior experience working within a regulated industry where government regulations influence process and behaviour is ideal, bonus points if you have experience in Home and Community Care, ideally within Aged Care.

As the Team Lead, you will need to have strong leadership and coaching skills and the ability to support a growing team - Your team will come second to none.

Finally, you'll need to be able to roll up your sleeves and have a strong desire to learn and grow in a fast paced, health-tech scale-up environment, with the ability to adapt to a fast changing business environment.
