**Your Role**

Reporting directly to the Clinical Manager, you will be responsible for leading the delivery of high quality support and experience to HomeMade customers by fostering good relationships with customers, and their support network while providing consultation, advice and case management support.

Given HomeMade is a relatively young business offering something new in the HCP marketplace, you will have the ability to identify opportunities for service innovation and take a proactive approach to implementation whilst the business continues to grow rapidly - and
**we aren't kidding** when we say rapidly.

**_Some of your responsibilities will include and are not limited to:_**
- You will be a primary point of contact for consumers, informal carers and service providers in the HomeMade network.
- Undertaking a collaborative and goal orientated approach to developing and updating support plans with customers.
- Maintain oversight over customers outcomes and changes in circumstances.
- Provide support with clinical supervision, assessment and recommendations for customers accessing HomeMade services.
- Act as a consultant to the Support and Services team to identify and articulate medical needs and interventions.
- Upholding and delivery of services in accordance with the Aged Care Act and Aged Care Quality Standards.
- Participate in, contribute to and implement quality improvement and risk management into all aspects of service.
- Working closely with customers and Service Providers to onboard the customer and their team to self-management.
- Support the Clinical Manager in monitoring the standards of support management within the growing Customer Support team.

To be successful in this role, you will be a Registered Nurse with demonstrated experience within the Home and Community Care space, ideally focusing on Aged Care.

Having a well-developed understanding and the ability to delivery clinical support and case management is essential.

Customer service will sit at the heart of what you do, so you'll have strong communication skills and the ability to work closely with our HCP Customers.
