**About the role**

This full-time, remote role provides the flexibility to work from home, with in-person visits to clients.

As a Support Coordinator, you will work closely with clients to help them effectively use their NDIS-funded supports, connect with appropriate service providers and allied health professionals, and track their progress toward achieving their goals.

**Responsibilities include**:

- Support clients and families in accessing and coordinating services.
- Build relationships with internal and external stakeholders.
- Implement supports outlined in NDIS plans.
- Help clients develop skills to coordinate support and engage in the community.
- Empower clients to understand and implement their plans.
- Be responsive to client needs, especially in crises.
- Conduct regular progress reviews and report to the NDIS.

**Skills and Experience**:

- Understanding or knowledge of the NDIS
- Experience in support coordination or similar
- Familiarity with psychosocial disabilities (advantage)
- Excellent interpersonal skills and the ability to collaborate effectively
- Strong written communication skills
- Demonstrable problem-solving skills

**Requirements**:
The successful applicant will have or be willing to obtain
- Driver’s Licence
- Working with Children Check
- NDIS Worker Screening Clearance
- Relevant qualifications in disability services, social work, or related fields (desirable)

While we are not yet an established Support Coordination provider in NSW, we are rapidly growing and excited about expanding our reach and services in the region.
