We are super excited to be seeking a friendly Support Coordinator that are passionate about care We are an online home and community care company. We are looking for a motivated individual with experience working with participants, Nurse with a smiles motto "creating community together". We believe each individual is vital in creating a safe and communicative space for all. Support Coordinator skills include: - Providing advice and consultation by making phone calls to participants, families and external service providers; - Preparing reports in conjunction with relevant stakeholders; - Liaising with colleagues and discussing service options for participants; - Performing Support Coordination to assist participants in implementing their NDIS plan; - Coordinating services in participants' NDIS plans, including the management of multi-disciplinary teams - Communication - Collaborate with all stakeholders to keep them informed - Interpersonal skills - Project management skills - Coordination of services with multiple stakeholders - Time management - Excellent verbal and written communication skills. - Excellent critical thinking and organisational skills. - Ability to build rapport and trust with stakeholders. - Ability to problem-solve - Strong time management and ability to prioritise. - Excellent knowledge of MS Office and other basic computer programs. - Ability to work as part of a team and maintain good working relationships. - Ability to manage the training of staff in organisation and NDIS requirements Support Coordinator must have; - Car with comprehensive insurance - NDISWC completed, NDIS Module, PPE Module (preferred) - Smart phone - Police clearance - 6months experience prior in same role **Job Type**: Casual **Benefits**: - Travel reimbursement - Work from home Schedule: - Day shift COVID-19 considerations: We have covid 19 management protocols in place to protect participants and staff.
