The HMHB team are an integral part of the Child and Family program within Monash Health Community. The team is comprised of Social Workers, Community Development Workers and a Nurse/Midwife. We are committed to providing women with high quality care, support, education and advocacy throughout their pregnancy and following birth, which embrace family centred, strengths based and evidence-based practice, within a Social Model of Health. **A Day in the Life**:
This is a great opportunity for a passionate dedicated Outreach/Case worker seeking to work in a creative and rewarding role in a supportive growth environment. Support pregnant women experiencing psychosocial complexity to improve the health and wellbeing outcomes for themselves, their babies and families. Based in Springvale, this exciting outreach role supports clients across Shire of Cardinia, Greater Dandenong and Casey Local Government areas.

As a Monash Health team member you will have the ability to maintain a high level of communication with clients, colleagues and other internal and external care providers, enabling you to deliver excellent care.
- You will provide a comprehensive range of care and services to pregnant women and their families experiencing complex vulnerability. Support is provided through an outreach model of care, supporting women within the hospital setting and the community including their homes, outpatient clinics, and other community venues.
- You will provide excellent Psychosocial and risk assessment, assisting women to develop goals and access appropriate community supports to address risk factors and improve the health and wellbeing outcomes for themselves and their babies.
- Work across a variety of clinical specialisms providing evidence based clinical Social work to women who are pregnant and until approximately 6-8 weeks post birth.
- Enjoy a fulfilling role working both independently and collaboratively with a fantastic group of people who are inclusive, passionate and share a vision to provide excellent, evidence-based care for our clients and their families.
- This position is based in Springvale, however, regular travel to other sites in the South Eastern region will be required. Program vehicles are provided for outreach and client transport.

- Qualifications and relevant paediatric experience as a Community Development worker who has worked with women and families who provides a strong focus on client centred care

**What you need**:

- Relevant Community Development/Welfare Qualification.
- Experience providing clinical care in a Paediatric or Women’s service is preferred
- Working with clients from Culturally and Linguistically diverse backgrounds will be highly regarded
- Please note a you will be required to complete a Police Check (or have valid Police Check)
- Working with Children’s Check (required)
- Current Drivers Licence without restrictions
- Appropriate Australian working rights
