About the role
As an Outreach Worker, you'll collaborate with Brimbank City Council and community-led organisations to deliver both outreach and onsite services. You'll be supported by the Brimbank Local Practice Manager and maintain connections with cohealth's Homelessness and AOD programs.
Key responsibilities include:
- Conduct assertive outreach across key locations to engage people experiencing homelessness and/or substance use
- Build therapeutic relationships, assess needs, and make warm referrals
- Coordinate care through person-centred planning, advocacy, and support
- Navigate service barriers such as stigma, literacy, or lack of awareness

- Strong understanding of homelessness and complex client needs
- A current Victorian Driver's License

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
