**The opportunity**

Our client is seeking an experienced and dedicated Care Manager to lead their team in south-east Melbourne.

- Flexible shifts from Monday to Sunday
- Full-time or part-time options available (minimum 4 days per week)

Key responsibilities include:

- Undertake allocated admissions, including complex cases.
- Utilise multiple approaches for clinical decision-making.
- Coordinate the care of clients and their carers.
- Establish therapeutic and professional relationships with clients and carers.
- Engage in collaborative practice with peers and team members to achieve client outcomes.
- Foster a supportive team environment and encourage professional development.
- Adhere to legislation and common law that impact nursing practice.
- Provide services in line with funding guidelines for project initiatives.
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years' experience in acute nursing
- Leadership experience as a Nurse In-Charge (desired)
- Excellent communication, organisational, and time management skills
- Proven ability to work independently and as part of a team
- Current driver's license and willingness to travel
- Current Federal Police Check
