Generalist Case Manager

**The Opportunity**

This Case Manager role is allocated across all the programs in the Child, Youth and Families including; Home Based Care, Kinship, Intensive Case Management Services and Targeted Care Packages (TCP) in a generalist role. The case manager would maintain a case load of mix portfolio, dependent on the need across the program area.

**Key Responsibilities**

The Case Manager role will be a generalist position within the Child, Youth and Families program. They will hold a caseload, providing case management intervention to provide preventative and secondary level support.
- Provide statutory case management services and care management to a range of children and young people residing both in out of home and parental care.
- To support young people, their families and Care Team in meeting statutory case planning goals, within the Best Interest Case Practice Model and Looking After Children Framework.
- Provide placement support, carer supervision and attending to all requirements to ensure carers are meeting accreditation requirements.
- Prepare reports as required for the Department of Families, Fairness and Housing.
- Ensure compliance is met within practice frameworks.
- Ensure the children and young people are linked with professional support and involve the children and young people and families in all the decision making.
- Coordinating care teams and liaison with key stakeholders and other service providers.

**About You**

You will have experience in outreach support and/or case management with young people. You require the capacity to understand, work and support young people, their family, carers and networks within a community environment and the skills to engage, negotiate and advocate.

You will require a completed tertiary qualification in Community Services, Education, Social Work or similar field. You will be a quick learner with high energy and commitment to your clients, yet able to work within the parameters of the role. You will need to be able to work flexible hours to provide a responsive service to your clients.

Experience undertaking contracted case management within DFFH, Child Protection or another similar agency will be looked on favourably.
