Guide Dogs Victoria has a full time permanent opportunity for an experienced professional to join our team as a Support Coordinator. The role will be a primary support for clients who hold funding packages, to assist the planning for the use for their funding and implementing the services that our clients require.
In a nutshell, the role of a** Support Coordinator** is to facilitate greater independence in all aspects of a client’s life by exploring their individual needs, what services and support they require to achieve their goals and how these services can be accessed. You will work proactively with participants by coordinating support to ensure they are getting the most out of their NDIS plans, reducing barriers and putting action plans in place along the way for them to live more independently and be included in the community.
**In this role you will be responsible for**:
- Supporting clients to understand their funding and assist them in defining an action plan for identified goals;
- Working with clients to implement a variety of support options in line with their NDIS plan;
- Ensuring that mainstream services are meeting clients requirements;
- Ensuring funding supports are being implemented within the allocated budget;
- Connect participants with appropriate service providers and coordinate services to meet participant’s needs.
- Provide information and support to participants regarding their NDIS plans and funding.
- Develop and maintain positive relationships with participants and their support networks
- Excellent interpersonal skills, with the capacity to liaise effectively with a wide range of individuals and service
- Support participants to access the community and achieve their goals. Manage administrative tasks related to the coordination of NDIS plans and funding.
- Assisting clients in building greater independence to be able to self-direct services and supports in the future;
- Providing NDIA with reports on outcomes and success indicators; and
- Evaluating the clients’ progress against identified goals.
- Qualifications in social work, health or social science, Allied Health, Cert IV in Community Services or Diploma in community work or equivalent
- Exceptional communication skills;
- Experience as an NDIS Support Coordinator preferred
- Excellent interpersonal skills, with the capacity to liaise effectively with a wide range of individuals and service providers
- Ability to be able to navigate the multiple sectors to link individuals to relevant services and support;
- High level understanding of the support and services provided by Guide Dogs Victoria;
- A strong client and customer focus and a commitment to person centred service practices;
- A commitment to live the values and behaviours of Guide Dogs Victoria; and
- A strong alignment with the vision of Guide Dogs Victoria in delivering independence to people who are blind or have low vision.
