**About the Role**

You will be the face of our organisation and will have a person-centered approach to client support.
Your main responsibility will be to meet with participants of the NDIS to create and implement support plans to help achieve the NDIS participant’s goals.
- **Build meaningful, long-term relationships** with clients and their families, ensuring their evolving needs are understood and met with compassion
- **Lead the coordination and creation of tailored care plans** for clients across both Disability and Aged Care services, putting individual goals at the center
- **Collaborate closely with multidisciplinary care teams** to ensure the seamless delivery of high-quality, person-centered support
- **Be the trusted first point of contact**—responding to inquiries, resolving concerns, and managing service requests with confidence and care
- **Advocate for our clients**, ensuring their voices are heard and their choices are respected every step of the way

**About you**
- **Proven experience in client-centered care and engagement**—if you've worked as an NDIS Client Coordinator or Support Coordinator, that's a big plus
- **Strong knowledge of care plan development and review processes**, with a focus on achieving client goals
- **Solid understanding of the NDIS Framework** and various Community Services Funding Models
- **Hands-on experience implementing Behaviour Support Plans** in line with best practices
- **Up-to-date knowledge of current NDIS legislation** and how it impacts service delivery
- **Ability to manage and maintain a diverse client portfolio** with confidence, compassion, and professionalism
