We are looking for an experienced and passionate individual who thrives on making a difference to the lives of older West Australians. Bethanie Home Care provides a wide variety of services to clients, covering day to day living needs in their own homes including: personal care, domestic and gardening services, respite services and social support in Geraldton, Perth and the South West. As a **Case Manager,** you will be responsible for coordinating a dedicated team of staff who are passionate about providing in-home care for our clients, based on a holistic reablement and wellness approach.
We are seeking an enthusiastic, energetic and highly flexible individual to join our Bethanie Home Care - North Metro team, **on a 12 month Fixed-Term contract** basis. Supporting areas from Yanchep, Banksia Grove, Burns Beach, Wanneroo and all suburbs in between. You are required to be available from **Monday to Friday, 8:00am - 4.00pm**.
**What will your day as a Bethanie Home Care Case Manager look like?**
- Partner with our lovely clients and their families, providing excellent communication, ensuring choice and dignity are maintained at all times
- Provide a person centred approach to assess, identify and document client needs, preferences and diversity
- Work collaboratively as part of the multi-disclinary team to ensure all care and referrals for clients are appropriate and timely
- Provide case management support and conduct regular reviews to ensure client requirements and expectations are being met
- Conduct and/or coordinate ongoing reviews and assessment, including client budget assessments, in accordance with the Home Care Agreement
- Actively seek and manage feedback from clients to ensure continuous improvement
- Use AlayaCare (software) to clearly document all client contact, assessments and activitites and communicate outcomes and updates to care plans with relevant team members
- Develop and maintain relationships with local networks and external agencies to enhance access to services and supports for daily living for our clients
- Work collarboratively with the clinical team and regional manager to identify and manage clinical needs to support best care outcomes
- Keep abreast of legislative and service changes and communicate these to clients and team members
- Willingness to uphold the Mission, Vision and Values of Bethanie
- Demonstrated knowledge and experience within a person centred care context provided in the
Aged Care and Home Care industry
- Thorough knowledge of Case Management principles
- Experience developing and managing client budget
- Ability to work within a team, as well as autonomously
- Ability to lead and develop a small to medium sized team
- Enthusiasm for the provision of excellent customer focused services, person centred care
and empathy for our clients
- Excellent verbal and written communication skills.
- Well-developed computer skills, with the ability to confidently use Microsoft Word, Excel and
database systems. Experience with AlayaCare would be highly advantageous, however not essential
- Knowledge of Continuous Quality Improvement and Occupational Safety and Health
requirements
- Current drivers licence and access to a reliable vehicle
- Willing to obtain required vaccinations
