Case Manager

Our Bethanie Community team are seeking a highly motivated individual that is looking to further their career and gain valuable leadership experience in a **Case Manager position.** This position is open to a candiate with a strong background in a Case Manager or Allied Health/ Registered Nurse role.

The Case Manager is responsible for care coordination for our community clients within a holistic reablement and wellness approach. This enables us to enhance the lifestyle and living experiences of those residing at home. The Case Manager will participate in the implementation of a client directed model of care which focusses on empowerment of the client to maximise quality of life, promote healthy active ageing and facilitate community re-engagement.

Bethanie will provide support, training opportunities and give you the tools to operate as an effective Case Manager. Ideally you will be innovative and committed to driving a culture of excellence and quality service to Bethanie’s clients.

You are ably supported in your role with a Clinical Nurse, RN, EN, OT & Physiotherapist and a great Regional Manager. You will share the 80 clients with the other case manager, you will manage approximately a case load of 35-40 clients personally.

This role is being offered on a **Permanent Full-Time basis**, **working 38 hours across Monday - Friday (8am - 4pm)** and will require an individual who is willing and available to be on call after hours and on weekends, if the need arises.

**About the Role**:

- Client and goal centred approach to care planning, incorporating regular and structured review processes to ensure currency and appropriateness of care plans is maintained
- Develops sustainable relationships with local networks and external agencies to strengthen access to social opportunities and lifestyle options across the service footprint
- Responsible for ensuring all client budgets are monitored and managed in line with approved package level and individual client requirements
- Be an effective member of the multi-disciplinary team, to ensure effective and holistic care is delivered to all clients.
- Provides reports, information and support as requested by the Regional Community Manager to a highly quality and in accordance to policy and procedures

**About You**:

- Strong background in Case Manager position, Allied Health or Registered Nurse experience would be an advantage
- Experience managing a team of care providers
- Excellent customer service, negotiation, communication and support skills
- Excellent verbal and written communication skills and strong computer skills and experience with client database systems
- Valid driver’s licence and access to a reliable vehicle

**Desirable**:

- Holds current registration as an Allied Health professional or Registered Nurse with AHPRA
- Knowledge of the Home Care Packages Programs, Commonwealth Home Support Program and Aged Care Standards
- Demonstrated experience in the Aged Care arena
