**About the role**
This position is integral to implementing changes to the funded services over the next 18 months.
You will be responsible for management of a team of Community Workers and volunteers to ensure that people with disabilities, people over 55 years and their families receive the support needed to be healthy, included and valued in society.
You will collaborate with another Coordinator, overseeing and driving achievement of both the operational and financial outcomes of a wide range of CHSP, HCP, NDIS services and Council activities.
On a daily basis you will:
- Ensure staff and volunteers provide best practice support, planning and advocacy to achieve each individual’s goals and successful group activities such as person-centred planning, teaching skills, wellness, positive behaviour support, solution focused approach and positive and healthy ageing
- Monitor the activity KPIs, data integrity and records to ensure accuracy
- Ensure compliance with funding agreements, internal and external standards, policy and procedure and legislation
- Support and lead the team, work with the broader Community Care team
**About You**
- Extensive experience working with people with disabilities and people over 65 years
- Minimum 5 years’ experience in coordinating services and staff in the community sector using a person-centred and/or wellness approach
- Degree in community work discipline or related area
- Ability to think and plan strategically
**What’s on offer**
We are offering an attractive total remuneration from $86,409.73 to $95,156.96 Plus Superannuation.
