The Supporting Accommodation for Vulnerable Victorians Initiatives Team (SAVVI) work collaboratively with the SRS residents, Proprietors, internal and external stakeholders.

**Job Description**:

- ** Full Time Opportunity - 12 Month contract with potential for extension**:

- ** $39.62 - $43.34 p/h + Super + Generous **Salary Packaging** benefits of up to $18,550**:

Working directly with residents in SRS accommodation, as well as proprietors you will;
- Identify unmet resident health and social needs and assist them to access appropriate supports and services
- Build skills/capacity of proprietors and staff to identify resident needs
- Improve care planning to meet those needs and to better manage challenging behaviours
- Build skills/confidence of residents and assist with negotiating and advocating to access services and participate in activities as independently as possible
- Strengthen links between pension-level SRS and local services, including developing new service approaches
- Manage flexible funds to assist residents to access services and activities Integrate and complement the identified SRS’s goals and expenditure alongside the SAVVI Partnership Coordinator position

You will also collaborate with internal and external stakeholders to build pathways to other programs, in-house and within the community. The Supporting Connections Team works collaboratively with the residents and Proprietors in the SAVVI program, as well as internal & external stakeholders. You will also work alongside the Partnership Coordinator and other committed members of the SAVVI team.

**Qualifications**:
EACH currently provides a range of services and supports under the 'Supporting Accommodation for Vulnerable Victorians Initiative (SAVVI). We are looking for a **Full time** outreach worker, with** tertiary qualifications in health promotion, community development, a health or human services discipline**, with demonstrated skills and experience in capacity building, development of SMART goals, assessment, care planning and report writing.

You will bring a strong understanding of the overarching emphasis and importance of capacity building of the Proprietors and staff of the SRS to set up their own resources and networks for their SRS and residents that they will be able to maintain and build on in their future, as well as understanding of the principles of community development.
